Job Responsibilities:	1.Initiates and oversees activity operations to ensure resident satisfaction and the proper completion of activities within a timely manner. Encourages resident participation in programs that meet the resident�s mental and physical needs. 2. Assists in the planning of effective individualized programming through written assessment, multi-disciplinary treatment participation and completion of required documentation according to established guidelines. 3. Assists in Providing goal-oriented and appropriate group and/or individual sessions designed to meet the needs of all residents and completes documentation on activity plan designed for each resident in a timely manner following established guidelines. 4.Prepares the display activity calendar to ensure that residents are participating in planned activities and aware of the activities that are available to them. Assists Activities Director in the development of programs that would best meet these individuals needs as well as continuing to meet the needs of other residents. 5. Demonstrates accountability for assessing, updating and maintaining resident skills through activities. Attends departmental meetings, in-service programs and sets annual professional goals. 6. Initiates activity charting and development of a activity program to meet resident�s physical and mental stimulation needs upon admission and continues to monitor residents weekly and completes charting as required. Assists in developing innovative new activities, programs and entertainment based upon the needs of all residents within the facility. Works with other departments to organize activities and assists in discussing new activity ideas with department managers. Assist in developing programs based upon resident needs. 7. Ensures that the necessary supplies and equipment is arranged for in a proper and timely manner prior to the activity. Works with team members to set-up for programs, gather residents and guests, initiate and encourage participation in activities, and clean up after each activity is completed. 8. Welcomes and prepares for new residents, visits residents on special occasions, writes and creates written and visual displays for resident appreciation. Monitors activity in the activity center and keeps work area clean and free of anything that could be harmful to residents. Assists in decorating or set-up for special events. 9. Seeks guidance and direction as necessary for the successful completion of job duties, utilizes the appropriate chain of command and keeps immediate supervisor aware of any expected changes and potential problem areas in a timely manner. 10. Assists in applying standards of performance improvement to exceed the needs of the residents and improve services. Participates in departmental performance improvement activities. 11. Performs other duties as assigned.

Job Responsibilities:	1. Provides patient care related support including services relative to hygiene, nutrition, ambulation, elimination and safety as directed by Charge Nurse per established departmental guidelines and competencies. 2. Documents appropriately into patient record charting based on skill level and follows established standards and guidelines of the nursing department. 3.Upon admission, accurately completes personal belonging sheet, obtains and records height, weight and vital signs, orientates new resident to the facility and unit, ensures that resident receives a arm band and provides resident with a labeled admission kit. 4. Turns and repositions residents every 2 hours. Repositions seated residents every hour. 6. Takes vital signs and weights accurately and as instructed by Charge Nurse and reports to charge nurse promptly and accurately. Ensures that proper slings, splints, braces, positions devices, specialty care items, etc. are applied according to physician or therapy orders. 7. Accurately measures intake and output on the I & O sheets and relay information to Charge Nurse at shift change. Performs procedures and treatments as instructed by Charge Nurse (i.e.: Sitz bath, moist packs, isolation techniques, etc) 8. Maintains appropriate supplies and equipment on the unit and alerts charge nurse or purchasing clerk of additional needs. Ensures that charge items are entered according to departmental procedure. 9. Successfully completes job duties by attending continuing education classes and in-services, accepting reassignments to other areas, taking on increased responsibility and keeping Charge Nurse informed of pertinent information that may require follow-up. 10. Demonstrates unit-defined competencies as required. 11. Interacts and maintains effective working relationships with other team members, charge nurse and department manager. Maintains discretion in handling confidential information. Gives report to charge nurse at the end of each shift and completes walking rounds with the oncoming or leaving shift.

Job Responsibilities:	1.Responsible for completing all MDS, admission, significant change, discharge, re-entry tracking forms quarterly and annual assessments. Responsible for the completion of all Resident Assessment Protocol and entering all necessary information into the computer system. Maintains records of all MDS/RAPS for a 15-month period. 2.Ensures all departments involved in the Resident Assessment Instrument (RAI) process complete their MDS sections/RAPs and Care Plans in according to the time line set by the MDS Coordinator to stay within state guidelines. Prepares and distributes time lines and calendars to all departments involved in the RAI. 3.The MDS Coordinator must schedule in conduct care plan meetings for all residents. Sends out letter to all residents� families for notification of care plan meetings and schedules according to their availability. 4. MDS Coordinator successfully completes a preliminary care plan on all new admissions within in 72 hours of admission. Responsible for completing comprehensive patient specific care plans on all residents every 90 days. Updates resident care plans as needed with necessary changes. Backs up computer system weekly to ensure proper transactions. 5.Coordinates all departments involved in the care plan process complete patient specific care plans in a timely manner and in accordance with the regulations set by the Division of Aging. 6.Archives all MDS assessments and transmits them successfully to the Division of Aging. Minimal of every 30 days - preferably weekly. 7.Notifies supervisors and charge nurses of any concerns, problems or updates professional and in a timely fashion. 8.The MDS Coordinator must conduct chart audits on all admissions and re-admissions for accuracy, competency and ensures that entire admission/re-admission process was completed as outlined by nursing policies. Audits all physician telephone orders for accuracy and ensure that they are completed properly and timely. 9.Monitors and tracts all facility infections. Ensures that infection control forms have been completed on all residents on antibiotics. Follows up on all resident cultures and input all diagnosed infections in to the computer system. Compiles monthly, quarterly and yearly infection control reports. Computes infection % per every 100 residents monthly. 10.Works with physicians, team members, other departments to ensure that they are aware of the completeness and accuracy of the assessments. Coordinates written plan. Ensures that the care plan includes measurable objectives and timetable to meet residents medical, nursing, mental and psychological needs as identified in the resident assessments. 11.Demonstrates flexibility in responsibilities and professional competency at all times. Participates in in servicing, continuing education and other activities to keep professionally current. Maintains working knowledge of emergency procedures and demonstrates awareness of safety issues relevant to patient and employee safety. 12.Communicates effectively with residents, visitors, co-workers, vendors other team members using professionalism. Collaborates with DON and other departments for problem solving and communication of resident related issues the DON. 13.Serves in on-call rotation and effectively resolves problems within the nursing department that occur when on-call in a professional and timely manner. 14.Completes other duties as assigned.
